Pet Grooming Release Form
Our furry friends are important to us. We care, so we want to assure you that ever effort will be taken to make your pets visit as pleasant as possible. Due to unpredictable behavior of animals, situation may arise that can be unexpected. Please read and sign that you understand all of the following:
Medical Conditions: Occasionally, grooming can expose hidden medical problems, or aggravate a current one. I understand that this may occur during or after grooming.
Senior Pets: Senior pets (8+ years and about) may have higher risk of stress, soreness, and injury during the grooming process. A special handling fee may be applied. All dogs of senior age must sign our Senior Pet Release.
Matted/Tangled Pets: When pets are severely tangled or matted they are at greater risk of injury, stress, and trauma. All precautions will be taken. However, problems can arise. These can happen during or after grooming and include: clipper irritation, stress, underlying skin condition, nicks/cut, hematoma, and bruising.
Late Arrival, Cancellation and No-Show Policy
If you are running late, please call us. Your appointment time slot will be held for a maximum of 15 minutes past booking time. If you arrive later than 15 minutes past your booking time, your appointment will then be at the groomer/bathers discretion to continue late service or reschedule. If your groomer/bather accepts late appointment, a $15 late fee will be applied. If your appointment has to be canceled due to tardiness a charge of $25 dollars for reserved booking will be charged.
Please provide a minimum of 48 hour cancellation or rescheduling notice. This allows time to rebook the appointment spot. Late cancellations/reschedules will be subject to a $25 dollar fee. Same day cancelations subject to a 50% of reserved booking fee. We understand things come up. We always excuse the first incident, and emergencies.
No Shows will be charged 50% of reserved booking. Two or more no shows will result in 100% charge of reserved booking when making future appointments.
Prompt pick up is appreciated and suggested. Pets left in our care for an extended period of time after pick up text/call could result in fee.

-Due to the nature of pet grooming all prices quoted prior to grooming are only estimates. This price can change during groom due to behavior, temperament, special handling, age of pet, or condition of coat.
-Doggie Styles has the right to refuse service at any time.
-I can be liable for medical expenses and damages that result from injuries caused by my pet. Including to groomer/bather and to equipment.
- If any medical problems arise while my pet is in the care of Doggie Styles Day Spa, I authorize Doggie Styles to do whatever is necessary for the safety, health and wellbeing of my pet (Unless opting otherwise in other release forms). I assumes full financial responsibility for any and all expenses.
-I declare my pet has not been exposed to ANY infectious disease. Should they be I will let Doggie Styles Day Spa know before scheduling.
-My pets nails will be clipped and filed. There is a possibility of exposing the nails quick, which may or may not bleed at time of grooming or after. If this occurs at appointment we will make you aware and send pet home with styptic powder. I am aware that although quicked nail may not have been present at groom, running or walking on rough surfaces after nails have been done can expose and quick nail.
-When booking Skunk Treatment, Flea Treatment, Touch of color, Express Groom, and Hand Scissor add-ons I must book when setting up appointment to ensure proper time for service.
-All pets are required to be on leash when on property
-If my pet is found to have fleas or skunked. My pet will be treated and a fee will be charged.
-We ask you take your pet on a bathroom break before drop off. Accidents resulting in rewash/drying can result in a waste cleanup fee. I agree to allow Doggie Styles Day Spa to take my dog outside should it need a bathroom break.
-My pets photo may be taken and shared to our social media outlets
-Any issues with my pets groom Must be reported within 48 hours after groom, in order to be addressed and corrected.
- We do ask ask 4 to 6 hours for grooming. I understand that this is just an estimate. We will call or give you a heads up when pet is/will be ready. If you show up earlier we cannot guarantee pet will be finished, full price of groom will be charged.
-I understand Doggie Styles Day Spa has the right to refuse service. If we feel the safety, wellbeing, or general respect of an animal or staff member are in jeopardy.
Doggie Styles Day Spa employs extensive safety and sanitary precautions to ensure your pets safety and health. Despite efforts on rare occasion pet may experience side effects of the grooming process, including but not limited to: razor burn, quicked nail, reaction to products, hair splinters, hematoma, stress, bruising, and exposure of health conditions. By signing this agreement you (the client) agree to relieve Doggie Styles Day Spa, and its agents, of any and all liabilities, and/or costs associated with any veterinary care, symptoms, or discomfort of your pet related to or presenting after services rendered at Doggie Styles Day Spa. The pursuit of any veterinary care, behavioral, or rehabilitation, and the payment thereof after, is the sole responsibility of the pet owner. Additionally, you agree to relieve Doggie Styles Day Spa and its agents of any and all liability associated to person, pet, or property by or relating to services rendered or service equipment used by Doggie Styles Day Spa and its agents.
